Download the Merchant application here. Complete Sections 1, 2, 3, 4, 5, 6, & 9 of the application. DO NOT sign it, and leave Sections 7 & 8 blank.

Email your application to merchantapp@paysteps.com, along with the following:

• If currently accepting cards, last 3 months merchant processing statements
• Last 3 months bank statements
• Voided check or bank letter showing your account name and number
• Copy of Driver’s License from each individual with 25% or more ownership interest

Upon receipt, a sales representative will contact you to complete the application and return for your signature. For assistance with your application call 954-890-2418.
